Getting your treatment
Once you are ready for treatment, a Case Manager will work closely with your healthcare provider to coordinate treatment
Getting Started with Invivyd Care
Once your provider has made a decision to prescribe you an Invivyd treatment, Invivyd Care can provide assistance in facilitating your access to treatment.
Invivyd Care Case Managers are able to:
- Answer any questions you may have about how you can receive the treatment your provider has prescribed
- Help you and your healthcare providers understand coverage available through your insurance plan
- Review out-of-pocket costs for all patients and provide information about available co-pay assistance for commercially insured patients
You can initiate enrollment in Invivyd Care only after a decision to prescribe has been made by your provider. To initiate enrollment in Invivyd Care, fill out the Patient Enrollment Form online and submit using DocuSign or print the Patient Enrollment Form, fill it out, and provide it to your healthcare provider to return it via e-fax to Invivyd Care at 855-233-1706.
